Managing Fatigue During Thyroid Hormone Dose Adjustment

Helping you stay energised and emotionally balanced while finding the right dose

After thyroid cancer surgery, Hashimoto’s thyroiditis, or radioiodine treatment, the most challenging phase is often the “dose-finding” transition period:
feeling hot then cold, palpitations, shaky hands, wanting to sleep all day yet never feeling rested, brain fog, and emotional ups and downs.
These symptoms are almost always temporary, and the right supportive strategies can significantly reduce discomfort.

Morning: Gentle activation instead of pushing through

Start the day with a glass of warm salted water (36–38 °C with a small pinch of sea salt) to support adrenal function.

Take thyroid medication on an empty stomach, then within 30–60 minutes have a small serving of high-quality protein + healthy fat (e.g. boiled egg + half avocado, unsweetened Greek yoghurt + nuts) to prevent blood-sugar swings that worsen fatigue.

Daytime: Stabilise blood sugar — the hidden key to cutting fatigue in half

During dose adjustment, basal metabolism fluctuates dramatically and blood sugar can roller-coaster.

→ Eat every 3 hours, always including protein + healthy fat + a small amount of low-GI carbohydrate.

This simple rhythm dramatically reduces the classic afternoon “power outage”.

Afternoon low-energy dip: a 15-minute recharge ritual

Almost every patient feels the lowest energy between 3–4 p.m.

→ Lie down for 15 minutes with legs elevated above heart level + slow diaphragmatic breathing.

This improves cerebral blood flow and parasympathetic tone far more effectively than coffee.

Targeted supportive therapies (only under specialist supervision)

Random supplements during dose adjustment can be counterproductive. The three intravenous options our patients most frequently find helpful (always prescribed by the physician after blood review) are:

  • High-dose intravenous vitamin C → rapid oxygenation and reduced oxidative stress
  • Alpha-lipoic acid (ALA) → mitochondrial protection and improvement of cold extremities/brain fog
  • Glutathione → detoxification support and clearer thinking

(All intravenous therapies must be assessed and administered by qualified medical staff; self-administration is not safe.)

Night: Prioritise true restorative sleep

Switch off blue light after 9 p.m.

Before bed enjoy a warm milk or sugar-free high-protein drink with a small handful of magnesium-rich nuts, followed by gentle leg stretching and a warm compress to encourage deep sleep.

A gentle reminder for your emotions

“This discomfort is not weakness — it is simply my body finding its new balance.”

Keep this sentence visible; taking three slow breaths whenever you see it noticeably lowers anxiety.
