Shingles Vaccine (commonly known as “snake shot” or “herpes zoster vaccine”) is used to prevent shingles (herpes zoster). The most commonly recommended and used vaccine in Hong Kong (and globally) is the second-generation recombinant vaccine: Shingrix (Recombinant Zoster Vaccine, adjuvanted, produced by GSK).

Shingles is caused by the reactivation of the varicella-zoster virus (the same virus that causes chickenpox). After a chickenpox infection, the virus stays dormant in nerve tissues. When immunity weakens (due to aging, stress, diabetes, cancer treatments, immunosuppressive drugs, etc.), it can reactivate, causing a painful, unilateral, band-like rash with blisters along a nerve path. The pain is often described as burning, electric shock-like, or stabbing. The most serious complication is postherpetic neuralgia (PHN) — persistent nerve pain that can last months or years, severely impacting quality of life. Risk increases sharply after age 50; about 1 in 3 people will get shingles in their lifetime.

Main Uses and Recommendations

  • Prevents shingles and its complications (especially PHN).
  • CDC (US) and many international guidelines recommend:
    • Healthy adults aged 50 years and older.
    • Adults aged 19 years and older who are or will be immunocompromised/immunosuppressed (e.g., due to disease, organ transplant, chemotherapy, long-term steroids, HIV, autoimmune conditions).
  • Shingrix does not prevent chickenpox and is not for treating active shingles.
  • Even if you’ve had shingles before or received the older vaccine (Zostavax), Shingrix is still recommended (usually after a waiting period; consult your doctor).

In Hong Kong (as of 2026): Shingrix is the preferred vaccine. The older live attenuated vaccine (Zostavax) has lower efficacy (~50%) and is largely replaced. It’s not included in government vaccination programs or subsidies (unlike flu or pneumococcal vaccines), so it’s self-paid at private clinics/hospitals. The Scientific Committee on Vaccine Preventable Diseases is reviewing cost-effectiveness for potential future inclusion.

Common Form and Dosing
(for reference only — always follow your doctor's advice)

  • Shingrix: Intramuscular injection (usually in the deltoid muscle of the upper arm).
    • Standard course: 2 doses, each 0.5 mL.
    • For healthy adults: Second dose 2–6 months after the first.
    • For immunocompromised: Can be shortened to 1–2 months apart.
  • No annual boosters needed. Clinical data show >90% efficacy (up to 97% in some studies) in preventing shingles and PHN after completing the 2 doses, with protection lasting at least 7–11 years (long-term data suggest ~87–90% at 11 years).

Important Notes & Common Side Effects

Most common side effects (usually mild to moderate, lasting 2–3 days):

  • Injection site: Pain (~78%), redness (~38%), swelling (~26%).
  • Systemic: Muscle pain (45%), fatigue (45%), headache (38%), chills/shivering (27%), fever (21%), gastrointestinal upset (17%).
  • These are often stronger than with older vaccines but indicate a robust immune response; most people resume normal activities quickly.

Less common but serious:

  • Rare risk of Guillain-Barré Syndrome (GBS; very low incidence, ~3 extra cases per million doses per FDA monitoring).
  • Allergic reactions (seek immediate help for hives, swelling, difficulty breathing).
  • May temporarily affect daily activities (plan rest after shots).

Contraindications/Precautions:

  • Severe allergy to any component (including the AS01B adjuvant).
  • Acute illness or fever (delay vaccination).
  • Pregnancy or planning pregnancy (not recommended; Category C).
  • Breastfeeding: Discuss risks/benefits.

Co-administration: Can usually be given with flu, pneumococcal, COVID-19, or other vaccines, but confirm with your doctor.

Shingrix is a prescription vaccine and is available in Hong Kong through private hospitals, clinics, and medical centers such as Hong Kong Adventist Hospital, Gleneagles Hong Kong Hospital, 仁和医疗集团, EC Healthcare, and Virtus Medical Group.

The total cost for two doses is generally around HK$5,000–6,500, depending on the clinic, doctor consultation fees, and available promotions. Some packages may range from approximately HK$4,650–5,600, which typically include medical consultation and vaccination services.

A medical assessment is recommended prior to vaccination to determine individual suitability.

Individuals aged 50 or above, or those with higher risk factors, considering vaccination (such as prior Zostavax vaccination, chronic conditions like diabetes, concurrent medications, or concerns about side effects) are advised to consult a family doctor or specialist for personalized guidance.
